Your privacy

Chandler & Nightingale Dental is committed to protecting the privacy of our patients and everyone who contacts us. This statement explains what personal information we collect, how we use and protect it, and how you can see it, correct it or raise a concern.

Who we are

Chandler & Nightingale Dental is a dental practice at 40 St. Clair Avenue West, Main Floor, Toronto, Ontario. As a dental practice in Ontario, we are a health information custodian under the Personal Health Information Protection Act, 2004 (PHIPA), and we follow it in everything we do with your information.

Our privacy contact

Questions, requests and concerns about privacy go to our privacy contact, Dr. Michael Nightingale.

  • Phone: 416-927-0088
  • Email: reception@smilemail.ca (please mark it “Privacy”)
  • Mail: 40 St. Clair Avenue West, Main Floor, Toronto, ON M4V 1M2

What we collect

We collect only the information we need to care for you and to run the practice:

  • Contact details, such as your name, address, phone number and email.
  • Your health history, medications and allergies.
  • Your dental records, including examination notes, X-rays, photographs, treatment plans and treatment provided.
  • Insurance and billing information.

How we use and share it

We use your information to assess and treat you, to contact you about your care and appointments, to bill you and, with your consent, your insurer, and to meet our professional and legal obligations.

We share it only when we need to, and only as much as we need to:

  • With other health care providers involved in your care, such as a specialist we refer you to or a dental laboratory.
  • With your insurance company, when you ask us to submit a claim or a pre-determination.
  • When the law requires or permits it, for example to the Royal College of Dental Surgeons of Ontario.

We never sell personal information. We do not share it for marketing.

Your consent

When you come to us for care, we rely on your consent to use your information for that care and to share it with others who are caring for you. For anything else, we ask first. You can withdraw or limit your consent at any time, unless the law requires us to keep or share the information. If you withdraw consent, we will explain how that may affect your care.

How we protect it

We protect your information with physical, technical and administrative safeguards, including locked storage, password-protected systems with limited access, and privacy training for our team. We keep dental records for as long as the Royal College of Dental Surgeons of Ontario requires, then dispose of them securely.

If your information is lost, stolen or accessed without authority, we will tell you and, where required, the Information and Privacy Commissioner of Ontario.

Seeing or correcting your records

You have the right to see and receive a copy of your dental records. Please ask our privacy contact in writing. We will respond within 30 days, and tell you if we need more time. We may charge a reasonable fee for copies, as PHIPA allows, and will tell you the cost first.

If you believe something in your record is inaccurate or incomplete, you can ask us to correct it. If we do not agree to the change, we will explain why, and you can add a statement of disagreement to your record.

Questions or concerns

If you have a concern about how we handle your information, please contact our privacy contact first. We will look into it and respond.

You also have the right to complain to the Information and Privacy Commissioner of Ontario:

  • Information and Privacy Commissioner of Ontario, 2 Bloor Street East, Suite 1400, Toronto, ON M4W 1A8
  • Phone: 1-800-387-0073

Our website

Appointment requests. When you send an appointment request, the name, contact details and message you enter are sent to our reception team by email so we can reply. Please don’t include detailed health information in the form; we’ll talk it through when we reply.

Email. Email is convenient, but it is not a fully secure way to send health information. If you email us, we will take that as your agreement to receive email replies. You can ask us to use another way to contact you at any time.

Technical information. Like most websites, our site’s hosting service records basic technical information, such as your browser type and IP address, to deliver pages and keep the site secure.

Visit statistics. We use Google Analytics to understand how people find and use our website, for example which pages are visited, how visitors arrived, and whether someone tapped our phone number or sent an appointment request. Google Analytics uses cookies and receives information such as your IP address and device type. It does not receive the contents of the appointment request form. We use these statistics only in summary, to improve the website. You can block these cookies in your browser settings, or use Google’s opt-out add-on.

Maps and fonts. Our map is provided by Google Maps and some of our type comes from Google Fonts. When you visit pages that use them, Google receives your IP address, as described in Google’s privacy policy.

We do not currently use advertising trackers, or use Google Analytics for advertising. If we begin advertising online, for example with Google Ads, we will update this statement before we do.

Changes to this statement

We may update this statement from time to time. The date below shows when it last changed.
